Hereby declare that I am fully aware of all the risks involved in the keeping, handling, vaccinating, testing and treatment of my animals or birds, at an official quarantine station, and that I accept these risks fully.

I hereby indemnify the Department of Agriculture and the Directorate of Animal Health and any of it’s officials, against any loss or damage however caused as a result of keeping, handling, vaccinating, testing or treatment of my animals or birds undergoing quarantine.

I understand and accept that in the advent of my animal or bird being found to be infected with an exotic disease they will be destroyed at my expense forfeiting any compensation by, or claims against the Department of Agriculture.

I understand and accept that any loss incurred by me as a result of the failure of the heating system at or the electricity supply to the quarantine station, will be at my expense.

I understand and accept that in the event of my being unable to utilise the accommodation a the quarantine station for which I have already paid, the amount paid will be forfeited unless the relevant Quarantine Superintendent is notified in writing at least 30 days prior to the commencement date of the quarantine period reserved.
